We are excited to welcome you and your family for the upcoming 2026–2027 school year! Please follow the steps below to complete your child’s enrollment:


Steps to Complete Enrollment for the 2026–2027 School Year

  1. Go to the application webpage:
    https://apps.hisd-survey.org/surveys/enrollment
  2. Complete the application:
    • Fill out each section of the application.
    • Continue through all pages until you see the message “Finished.”
  3. Check for confirmation:
    • After submitting, you will receive a confirmation email.
  4. Fill out the additional Microsoft Form: https://forms.cloud.microsoft/r/Q5NY21ku7G?origin=lprLink .

  5. Wait for follow-up:
    • Once we receive your Microsoft Form, our team will contact you to schedule an appointment to finalize your child’s enrollment.

  6. Submit required documents (if not already uploaded):
    • If you did not upload documents during the application, please email them.


Required Enrollment Documents

  • Parent Identification
  • Student Identification
  • Proof of Address
  • Proof of Grade
  • Immunization Records or a letter from a doctor

     

Important: All documents must be submitted for enrollment to be considered complete.
